Schedule your FREE phone consultation
During this call you’ll speak directly with Dr. Jow, our sports medicine physician. While we can’t give out specific advice until you become a patient, we can discuss your issue and determine if Avid is the best fit for you.
If an in-person visit is recommended, we will schedule it right away.
Come see us for an in-person appointment
Our new in-person patient visits are 60 minutes long. You tell your story and your goals in your own words. We will review your medical history, lifestyle, habits and perform a detailed exam. This may include a diagnostic ultrasound to take a deeper look, if helpful, at no extra charge to you.

We will build a recovery program with you during the visit, so you can start ASAP. In many cases, our Athletic Trainer can begin therapeutic exercises and manual therapy on the same day.

Working with Dr. Jow on diagnosing and treating my acute bursitis was a patient’s dream. Her incredible bedside manner helped me feel comfortable and confident I was receiving the best care possible. And, after some physical therapy and two shots, my shoulder feels as good as new!
How billing works
We are currently out of network with private insurances. This gives us the freedom to:
- Spend more time with you.
- Individualize care without limitations.
- Create a unique care setting.
You can still use your insurance for tests or imaging that we order for you outside of Avid (ex- xrays, blood work). We collect payment at the time of service. We provide you with a bill to submit to your insurance provider and help you with steps to receive your out-of-network reimbursement, if allowed by your plan. You can quickly and easily submit our claim with Reimbursify.

I have a PPO plan. Could I receive a reimbursement for Avid?
Yes, if you have a Preferred Provider Organization (PPO) plan, you could receive a reimbursement! Your plan gives you the flexibility to go out-of-network for your care and may cover a portion of those costs. So once you hit your out-of-network deductible, you may receive a reimbursement for your Avid care.
What’s a deductible?
It’s the amount you pay up front for healthcare before your insurance kicks in. Let’s say that your out-of-network deductible is $250 and you haven’t spent anything yet. Once you hit $250 on an out-of-network provider like Avid, your insurance will cover a percentage of Avid appointments going forward.
I have an HMO plan, what does that mean for me?
I have an FSA/HSA. Can I use it on my Avid visits?
Yes! You can use your FSA or HSA for your visits with us. Flexible spending accounts (FSA) and health savings accounts (HSA) are programs that allow you to set aside money (before taxes) for specific health expenses. You can apply that towards your Avid appointments. If you have an FSA or HSA debit card, enter it as your payment source.
What types of payment do you accept?
We accept all major credit cards, FSA/HSA account cards, check and cash payments.
How do I submit Out of Network claims to my insurance?
